Who needs to be printed?
Anyone applying to be a Certified Nurse Assistant and Home Health Aide must be Live Scan fingerprinted.

 

What do you need to bring?

• The Certified Nurse Assistant /Home Health Aide Request for Live Scan Service form 

• Government-issued photo ID

• Your form of payment

Along with fingerprinting, each applicant needs to complete the Certified Nurse Assistant / Home Health Aide Initial Application.
